Family Emergency Contact Form

My husband and I recently had a babysitter that wasn’t a family member or friend. Instead of scribbling some phone numbers on a piece of paper, I figured I would create a one-page document that housed everything she would need in case of emergency.

We put the page in a protector sheet, and taped it to the inside of our medicine cabinet. This is a great resource to have on hand, as it dives into parent information, home information, as as well as a location for child(ren) stats.

I wanted to be able to share this resource, so I have made it accessible below! Note: you will need to utilize Canva to edit this document. Canva offers free accounts, and is very easy to use! All you have to do is download the PDF, then upload to Canva, and edit with your personal information!
